નિયોનેટલ ઇન્ટેન્સિવ કેર યુનિટ (એનઆઈસીયુ) વિશે મહત્વની જાણકારી.

બાળકનો જન્મ એક અદભૂત અને ખૂબ જટિલ પ્રક્રિયા છે. માતા અને બાળક બંને માટે ઘણા શારીરિક અને ભાવનાત્મક ફેરફારો થાય છે.

બાળકને માતાના શરીરની બહારના જીવનમાં ઘણી શારીરિક ગોઠવણો કરવી આવશ્યક છે. ગર્ભાશયને છોડવાનો અર્થ એ છે કે બાળક લાંબા સમય સુધી શરીરના મહત્વપૂર્ણ કાર્યો માટે માતાની રક્ત પુરવઠા અને પ્લેસેન્ટા પર નિર્ભર નથી.

જન્મ પહેલાં, બાળક માતાના કાર્યો પર આધારિત છે. આમાં શ્વાસ લેવાનું, ખાવું, કચરો દૂર કરવો અને રોગપ્રતિકારક શક્તિનો સમાવેશ થાય છે. જ્યારે બાળક ગર્ભાશય છોડે છે, ત્યારે તેની શરીર પ્રણાલીમાં ફેરફાર થવો જ જોઇએ. જેમ કે,

• ફેફસાંમાં હવા શ્વાસ લેવી જ જોઇએ.
• કાર્ડિયાક અને પલ્મોનરી પરિભ્રમણ બદલાય છે.
• પાચક તંત્રએ ખોરાક અને કચરાના વિસર્જનની પ્રક્રિયા શરૂ કરવી આવશ્યક છે.
• કિડનીએ શરીરમાં પ્રવાહી અને રસાયણોને સંતુલિત કરવા અને કચરો બહાર કાઢવાનું કામ શરૂ કરવું આવશ્યક છે.
• યકૃત અને રોગપ્રતિકારક શક્તિઓએ તેમના પોતાના પર કામ કરવાનું શરૂ કરવું જોઈએ.

તમારા બાળકની શરીર પ્રણાલીઓએ નવી રીતે સાથે કામ કરવું આવશ્યક છે. કેટલાક કિસ્સાઓમાં, બાળકને ગર્ભાશયની બહાર સંક્રમણ કરવામાં મુશ્કેલી થાય છે. અકાળ જન્મ, મુશ્કેલ જન્મ અથવા જન્મની ખામી આ ફેરફારોને વધુ પડકારજનક બનાવી શકે છે. પરંતુ નવજાત બાળકોને મદદ કરવા માટે ઘણી વિશેષ સંભાળ ઉપલબ્ધ છે.

નિયોનેટલ ઇન્ટેન્સિવ કેર યુનિટ (એનઆઈસીયુ) શું છે?

નવજાત બાળકોને કે જેઓ સઘન તબીબી સંભાળની જરૂર હોય છે તેવા હોસ્પિટલના ખાસ વિસ્તારને ઘણીવાર નિયોનેટલ ઇન્ટેન્સિવ કેર યુનિટ (એનઆઈસીયુ) કહેવામાં આવે છે. એન.આઇ.સી.યુ. પાસે નબળા દર્દીઓની વિશેષ સંભાળ રાખવા માટે અદ્યતન તકનીક અને પ્રશિક્ષિત આરોગ્ય સંભાળ વ્યવસાયિકો છે. એન.આઇ.સી.યુ. એવા બાળકોની સંભાળ પણ રાખી શકે છે જેઓ બીમાર નથી પણ ખાસ નર્સિંગ કેરની જરૂર હોય છે. કેટલીક હોસ્પિટલોમાં એનઆઈસીયુ માટેનો સ્ટાફ હોતો નથી અને બાળકોને બીજી હોસ્પિટલમાં ખસેડવું આવશ્યક છે. જે બાળકોને સઘન સંભાળની જરૂર હોય તે જો તેઓ એન.સી.સી.યુ. સાથેની હોસ્પિટલમાં જન્મ્યા પછી ખસેડવામાં આવે તો તેના કરતા વધુ સારી કામગીરી કરે છે. 

કેટલાક નવજાત બાળકોને એન.આઇ.સી.યુ. માં સંભાળ લેવાની જરૂર રહેશે. અકાળ બાળકને જન્મ આપવો એ કોઈપણ માતાપિતા માટે અનપેક્ષિત હોઈ શકે છે. આ માહિતી તમને એ સમજવામાં સહાય કરવા માટે છે કે બાળકને એનઆઈસીયુમાં શા માટે આવવાની જરૂર પડી શકે છે. તમને કેટલીક કાર્યવાહી વિશે પણ માહિતી મળશે જે તમારા બાળકની સંભાળ માટે જરૂરી હોઈ શકે છે.

કયા બાળકોને ખાસ કાળજીની જરૂર છે?

એનઆઈસીયુમાં દાખલ થયેલા મોટાભાગના બાળકો અકાળ જન્મેલા (નિશ્ચિત સમયથી બહુ વહેલો જન્મેલા) હોય છે, તેનું વજન ઓછું હોય છે, અથવા જેને ખાસ આરોગ્ય સ્થિતિની કાળજી લેવાની જરૂર હોય છે. જોડિયા અને ત્રિવિધ ઘણીવાર એનઆઈસીયુમાં દાખલ થાય છે કારણ કે તેઓ એકલા બાળકો કરતાં પહેલાં અને નાના જન્મે છે. આરોગ્યની સ્થિતિ જેવી કે શ્વાસની તકલીફ, હૃદયની સમસ્યાઓ, ચેપ અથવા જન્મજાત ખામીવાળા બાળકોની સંભાળ પણ એનઆઇસીયુમાં લેવામાં આવે છે.

નીચે કેટલાક પરિબળો છે જે બાળકને જોખમમાં મૂકી શકે છે અને એન.આઈ.સી.યુ.માં દાખલ થવાની શક્યતામાં વધારો કરી શકે છે. પરંતુ દરેક બાળકને એન.આઈ.સી.યુ.ની જરૂર છે કે કેમ તે જોવા માટે આકારણી કરવી આવશ્યક છે. ઉચ્ચ જોખમનાં પરિબળો નીચે શામેલ છે.

માતૃત્વ પરિબળો :

• 16 વર્ષથી નાની અથવા 40 વર્ષથી મોટી ઉંમર
• ડ્રગ અથવા આલ્કોહોલનો ઉપયોગ
• ડાયાબિટીસ
• હાઈ બ્લડ પ્રેશર (હાયપરટેન્શન)
• રક્તસ્ત્રાવ
• જાતીય રોગો
• બહુવિધ ગર્ભાવસ્થા (જોડિયા, ત્રિવિધ અથવા વધુ)
• બહુ ઓછી અથવા ખૂબ જ વધારે એમ્નિઅટિક પ્રવાહી
• પટલનું અકાળ ભંગાણ (જેને એમ્નિઅટિક કોથળી અથવા પાણીની થેલી પણ કહેવામાં આવે છે)

ડિલિવરી પરિબળો:

• ઓક્સિજનના અભાવને લીધે બાળકની અંગ પ્રણાલીમાં પરિવર્તન (ગર્ભની તકલીફ અથવા શ્વાસની તકલીફ)
• બાળકનો પ્રથમ સ્ટૂલ (મેકોનિયમ) ગર્ભાવસ્થા દરમિયાન એમ્નિઅટિક પ્રવાહીમાં પસાર થવુ
• ગર્ભાશયની દોરી બાળકના ગળામાં લપેટી (ન્યુક્લ કોર્ડ)
• ફોર્સ અથવા સિઝેરિયન ડિલિવરી

 

 બાળકના પરિબળો:

• 37 અઠવાડિયાથી ઓછા અથવા 42 અઠવાડિયાથી વધુની સગર્ભાવસ્થાની ઉંમરે બાળકનો જન્મ
• જન્મનાં સમયે વજન 2,500 ગ્રામથી ઓછુ અથવા 4,000 ગ્રામથી વધુ
• સગર્ભાવસ્થાની વય માટે નાનું
• જન્મજાત ખામીઓ
• ઝડપી શ્વાસ, કર્કશ, અથવા શ્વાસ બંધ કરવા સહિત શ્વસન તકલીફ (શ્વસન રોગ)
• લો બ્લડ સુગર (હાઈપોગ્લાયકેમિઆ)
• વધારાના ઓક્સિજન, IV (નસોમાં) ઉપચાર અથવા દવાઓની જરૂર
• રક્તસ્રાવ માટે ખાસ સારવાર અથવા કાર્યવાહીની જરૂર

Ectopic pregnancy

An ectopic pregnancy is a pregnancy that grows outside the womb, and can occur when the Fallopian tube is not functioning normally and the pregnancy grows within the tube (20 to 25 per cent of cases). However, in rare cases, it may occur on the cervix, in the abdomen or on an ovary. An ectopic pregnancy cannot survive as only the womb can support the growth of a foetus. If an ectopic pregnancy bursts inside a Fallopian tube, it is a life-threatening medical emergency and we are fully equipped to perform the urgent surgery that this requires.

લેપ્રોસ્કોપી એ તમારા પેટના અવયવો અને તમારા પ્રજનન અવયવોને નજીકથી જોવાનો એક માર્ગ છે. આ પ્રક્રિયાનો ઉપયોગ વિવિધ તબીબી સ્થિતિઓનું નિદાન કરવામાં અને બાયોપ્સી લેવા માટે કરી શકાય છે.

• તમારી શસ્ત્રક્રિયાના આગલા દિવસે મધ્યરાત્રિ પછી ખાવું, પીવું (પાણી સહિત) અથવા ધૂમ્રપાન ન કરો.
• શસ્ત્રક્રિયાના દિવસે લો હીલવાળા પગરખાં પહેરો. તમે એનેસ્થેસીયાથી તમારા પગ પર અસ્થિર હોઈ શકો છો.
• દાગીના ન પહેરો.
• ફિટિંગમાં ઢીલા વસ્ત્રો પહેરો. તમને શસ્ત્રક્રિયા પછી પેટની કોમળતા અને ખેંચાણ રહશે.

લેપ્રોસ્કોપી પછી શું થાય છે?

મોટાભાગના કિસ્સાઓમાં, તમે તમારી લેપ્રોસ્કોપી પછી ટૂંકા સમયમાં ઘરે જઈ શકશો. તમારી એનેસ્થેસિયા ન આવે ત્યાં સુધી તમારે રાહ જોવી પડશે અને તમારા doctor ખાતરી કરી લે કે તમે પ્રક્રિયાથી કોઈ આડઅસર અનુભવી રહ્યા નથી અને તમારી લેપ્રોસ્કોપી પછીના દિવસોમાં તમે ઘરે સ્વસ્થ થશો.

લેપ્રોસ્કોપી પછી એક મહિના સુધી યોનિમાર્ગ રક્તસ્રાવ થવો સામાન્ય છે. ઘણી સ્ત્રીઓમાં શસ્ત્રક્રિયા પછીના ચાર થી છ અઠવાડિયા સુધીનું તેમનું સામાન્ય માસિક ચક્ર હોતું નથી. જ્યારે તમારું સામાન્ય ચક્ર પાછું આવે છે, ત્યારે તમે કદાચ ભારે રક્તસ્રાવ અને સામાન્ય કરતા વધુ અસ્વસ્થતા જોશો. લેપ્રોસ્કોપીથી તમારી સ્થિતિને રાહત કરવામાં મદદ મળી છે કે કેમ તે નક્કી કરતા પહેલા બેથી ત્રણ માસિક ચક્રની રાહ જુઓ.
